Convenient On-Site Wellness Center

If you’re a little under the weather or just want to see how your blood pressure’s looking, a stop in our on-site Wellness Center may be just what the doctor ordered. Conveniently located in the Baker West wing, the Wellness Center is scheduled to open in Spring of 2023 and will be staffed by a nurse practitioner and a registered nurse every weekday.  Any resident can be seen the same day for acute needs such as an illness or minor injury. For those living with a chronic condition such as high blood pressure, arthritis, or weight control, health monitoring assistance is also available.

The center also offers several regularly scheduled services to meet the needs of residents. These include:

  • Daily delivery of prescription drugs from a local pharmacy
  • Regular blood pressure clinics
  • Flu and pneumonia vaccine clinic (seasonal)
  • COVID-19 booster clinic
  • Shingles vaccine clinic

Outpatient Physical Therapy Services

Whether recovering from an illness, injury or surgery, outpatient physical therapy services at Fox Hill Village provided by Connections Physical Therapy can help you get back to doing the things you love more quickly and safely. Expert, compassionate physical therapists work with you and your doctor to develop a plan that will help you regain strength and functional abilities. And best of all, you can return to the familiar comfort of your own home each day.

Inpatient Rehabilitation. Should you need inpatient rehabilitation, our licensed clinical social worker will assist you in choosing a quality short-term rehabilitation center within Norfolk County, MA, or the surrounding area. When you return to your home at Fox Hill, they will educate you regarding the on-campus health options that are appropriate for you and will help arrange any support services, private home health care, nursing care, or continuing outpatient therapies you may need.

Support Services: Making daily life easier

Needing a little help shouldn’t mean giving up the life you love. That’s why, at Fox Hill, you’ll enjoy the liberating feeling of knowing support services are available to provide personal care and assistance in your home. Our exceptional team of certified nursing assistants can help with activities of daily living, manage medications, and tailor a care plan that meets your individual needs and preferences.

Birch Grove: Assisted Living with Continuing Care

For those who find they can no longer live independently due to an advancing chronic condition that requires significant support services, assisted living with continuing care at Birch Grove is an exciting alternative to “the way things have always been done.” As an adopter of The Green House® Project small-house model, Birch Grove combines personalized assistance and care with the benefits of living in a community. Direct admission is possible for those who are not Fox Hill residents on a space-available basis.

White Oak Cottages: Memory Care and Support

White Oak Cottages is a unique alternative for those who can no longer live independently due to Alzheimer’s disease or another form of dementia or memory impairment. As a member of The Green House® Project, White Oak Cottages fosters a culture that honors and respects residents (called “elders”), caters to individual needs and preferences, and offers a physical environment that makes it easy for elders to move about freely and independently while also engaging in a vibrant community life.
