Whether you need short-term assistance recovering from an acute health condition or you simply find that certain activities have become more difficult to manage on your own, our support services team is here to help with in-home care designed to meet your needs and preferences.

We’ve got your back.

The Support Services team consists of registered nurses who oversee care; a licensed clinical social worker (LICSW) who coordinates resources such as home-health providers; and well-trained certified nursing assistants (CNAs) who provide daily assistance. But perhaps even more important than their credentials is the personal approach they take to caring for each resident. This is where the advantage of Support Services really shines through.

Your care team will work with you, your doctor, and your family to develop a plan for assistance that’s specifically tailored to you. They’ll also meet weekly with various departments at Fox Hill such as fitness and dining to determine how their services can best support you. And because the team that creates the plan is the same one providing care, they’ll get to know your overall health — which means they can proactively anticipate your needs and be on the lookout for early signs of any potential challenges. This level of personal attention can help you live safely in your home and can often help avoid a hospitalization.

How it works.

Support Services can be temporary for recovery from an illness, injury, or surgery, or ongoing to support you as your needs change. Services are provided according to the level of care needed — whether it’s minimal help with an activity of daily living such as bathing or dressing, more involved daily care, or care with comprehensive nursing oversight. Fox Hill’s team has close working relationships with several private care agencies who are familiar with our community and our staff. This creates the best possible scenario for residents who know the familiar Fox Hill team is overseeing their care.

Services provided by Fox Hill staff may include:

  • Assistance with activities of daily living (bathing, dressing, grooming)
  • Medication management
  • Mail and meal pick-up
  • Escort services for activities or appointments
  • 24/7 medical alert emergency response system

How do I know if support services are the right choice?

If you’re concerned about yourself or a loved one, it’s best to start with a physician’s assessment. Then, our support services team will help design a personalized care plan for assistance with daily tasks. This may include medication management; coordination of medical appointments; help with bathing, grooming, or dressing; additional help with housekeeping or meals; transportation and more.

Who pays for support services?

Residents or their families generally pay for this care from their own financial resources. Depending on the nature of an individual’s long-term care insurance policy, costs may be reimbursed.
