The Job:
We are seeking a Life Enrichment Coordinator (LEC) in our memory care cottages at White Oak Cottages (WOC). The LEC will have a vital role in supporting residents living with dementia by bringing intentional enthusiasm, warmth, and creativity to daily programming and interactions.
The LEC is hands-on and directly responsible for planning, leading and facilitating daily activities/programs that are meaningful, engaging and tailored to the evolving individual needs of the WOC community. This position requires a deep appreciation for the unique needs of individuals with cognitive impairment and a commitment to engaging residents with patience, flexibility, and joy.
Duties:
- Lead and facilitate daily personalized small and large group experiences by reviewing resident profiles and finding common needs and interests.
- Actively engage residents during programs and model enthusiasm, encouragement, and appropriate cueing with residents.
- Be physically present for scheduled activities and responsible for their execution from start to finish.
- Create monthly activity calendars for each Cottage in conjunction with the Director of Life Enrichment and the WOC Administrator.
- Work with the WOC Administrator and the Director of Life Enrichment, provide meaningful engagement training to CNAs.
- Work with outside entertainment vendors for programming and billing to help ensure invoices are received and submitted to the Director of Life Enrichment on a timely basis.
- Work within allocated budget for programming and supplies.
- Create and maintain an Activities Binder which will include the monthly calendar and a profile of each resident.
- Observe and record resident engagement using the Engagement Record and Enrichment Item Inventory tracking.
- Upon resident intake, complete the activity section of the PELI, the Enrichment Items Inventory, resident profile; and determine if any new resident enrichment items and group items should be purchased to accommodate new individual and group dynamics.
- Review Resident Engagement records weekly with the Director of Life Enrichment and Administrator to assess resident engagement and adjust activity calendar accordingly.
- Ensure the safety of all residents participating in programs both inside and outside the Cottages.
- Ensure that all activity supply areas and “self-starter” tables are kept neat, organized, and updated.
- Work with the Administrator and Director of Life Enrichment to train and provide guidance to volunteers.

We are seeking a Hospitality Security Manager to manage all aspects of our security operations. The Hospitality Security Manager will be responsible for ensuring that proper policies and procedures are in place to provide a professionally welcoming, safe, and secure environment for employees, visitors, assets, and confidential or proprietary information, in a cost-effective manner.
Duties:

The Job:
The Social Worker at Fox Hill Village (FHV) supports residents and families across the continuum – Independent Living, Assisted Living (AL), and Memory Care (MC) by providing comprehensive psychosocial support for residents in the community. The Social Worker will assist with admission assessments, coordinate mental health services, and educate and support residents and families with care transitions.
The Social Worker ensures that resident needs are identified, communication among care teams is smooth, and care transitions are handled with compassion. The Social Worker is instrumental in educating and navigating residents to resources both internally and externally.
Duties:
- Assists with screening prospective residents for admission to FHV Independent Living in a joint interview with a FHV nursing professional.
- Conducts psychosocial assessments and limited cognitive assessments.
- Obtains medical records, if necessary, and in conjunction with the Health Services Team, reviews applicants past and present medical history as part of the assessment process.
- Provides home visits to new residents to build rapport and educate them about health care services at FHV.
- Meets with residents for supportive intervention, information, and referral.
- If appropriate, offers brief counseling services for issues, including adjustment difficulties, family/interpersonal issues, mental health disorders, dementia or other neurological disorder, physical issues, and bereavement.

The Job:
We are seeking Per Diem Nurses (LPN or RN) at our premier senior living community to perform multiple care duties that contribute to the health, well-being, and quality of life for our residents.
Nurses are responsible for monitoring resident needs, notifying the Nurse Manager of any changes observed in resident condition, and ensuring that all services outlined in the residents’ service plans are delivered with excellent quality, on time and with respect.
Nurses assist with all aspects of compliance with all state and local requirements. Nurses report to the Nurse Manager and work in collaboration with the CNA’s.
Pay starts at $36/hour and there is opportunity for an increased hourly pay wage dependent on experience and credentials.
Duties:
* Engage in regular communications with the CNA work team on each shift to obtain an update on each resident
* Review and monitor overall adequacy of each resident’s Individual Service Plan (ISP), ensure that each ISP is being administered properly.
*Provide assessment visits to residents who present symptoms or appear to have had a change in their condition. Use nursing judgement to determine if any resident needs prompt medical attention, provide emergency nursing care/first aid such as CPR, Heimlich Maneuver, AED Machine as necessary.
*Communicate follow up care to CNA work team as appropriate, report incidents/illnesses to family members in a timely manner.
* Work in Collaboration with CNAs as part of the overall self-managed work team.
*Maintain communications with physicians and home healthcare agencies providing services.
