Staff Nurse | Fox Hill Village

Reports to:                 Health Care Director at Fox Hill Village 

Status:                        Non-Exempt (Hourly) 

Summary of Job Responsibilities:

The Nurse is responsible for assisting the Health Care Director in monitoring resident needs, notifying the Health Care Director of any changes observed in resident condition, and ensuring that all services outlined in the residents’ service plans are delivered with excellent quality, on time, and with respect.  The Nurse will assist with all aspects of compliance with all state and local requirements.  The Nurse reports to the Health Care Director and works in collaboration with the 

Nurses and CNAs in independent living as well as our assisted livings.   The nurse must support, honor, and respect the philosophy of the Green House® Project and its self-managed work team of CNAs. 

Essential Functions & Responsibilities:

  • Engage in regular communication with the CNA work team on each shift to obtain an update on each resident.
  • Review and monitor the overall adequacy of each resident’s ALIS Care Plan to ensure that each Care Plan is being administered properly.
  • Provide assessment visits to residents who present with symptoms or appear to have had a change in their condition.  Use nursing judgment to determine if any resident needs prompt medical attention; provide emergency nursing care/first aid such as CPR, Heimlich Maneuver, and AED Machine as necessary; communicate follow-up care to CNA work team as appropriate; report incidents/illnesses to family members.
  • Work in collaboration with CNAs as part of the overall self-managed work team.
  • Maintain communications with physicians and home healthcare agencies providing services.
  • Develop and implement strategies that promote person-centered care.
  • Work collaboratively with Social Work on identifying residents in the community who may benefit from one of our higher levels of care. 
  • Assist with assessments/ evaluations and care plan coordination as needed.
  • Assist with pharmacy medication delivery and observe or deliver medication in accordance with SAMM policies and procedures Evaluates residents for independent, SAMM.
  • Assist with in-service staff orientation including evaluating CNA compliance with SAMM protocol.  
  • Maintain emergency bag supplies and the safe condition of equipment, including PPE.
  • Monitor standard use of universal precautions.

Education and/or Experience Requirements:

  • Registered Nurse license or Massachusetts Licensed Practical Nurse license is required.
  • Current CPR certification is required. 
  • 2–5 years experience in LTC Skilled or Gerontological health care. 
  • Assisted living experience is strongly preferred.   

Physical Requirements:

  • Occasional lifting and carrying of equipment and supplies weighing 30 lbs. or less.
  • Occasional lifting assist of residents from the floor weighing up to 150 lbs.
  • Sitting 50%, walking, and standing 50%. Protective equipment as indicated.

CNA | Certified Nursing Assistant

Reports to:                 Supervisor, Health Care Services

Status:                        Non-Exempt (Hourly) 

Summary of Job Responsibilities:

The CNA is responsible to provide personal care services/ activities of daily living, medication reminders and light housekeeping to an elderly population resident at Fox Hill Village.

Essential Functions & Responsibilities:

  • Any task requested by management that helps to ensure resident safety and security.
  • Communicates effectively with residents.
  • Observes/documents and reports to nurse and next shift CNAs changes in resident status.
  • Maintains clean environment by performing light housekeeping duties as assigned by supervisor.
  • Provides personal care assistance (baths, dressing, toileting) in a professional manner while allowing the resident to maintain their dignity.
  • Assist in safe transfer and ambulation.
  • Participates in the safe delivery of medication reminders thru SAMM.
  • Personal laundry for residents
  • Responds promptly to radio calls.
  • Is aware of and practices reasonable infection control procedures.
  • Communicates ideas/issues promptly to supervisor.
  • Adhere to all safety policies including reporting on all incidents immediately to the supervisor, if the supervisor is not available report to Human Resources or another manager.

  • Ability to walk long distances throughout the facility.
  • Ability to perform repetitive motions such as bending, pushing/pulling, reaching, walking/standing, and climbing stairs.
  • Occasional lift assist of residents from the floor weighing up to 150 lbs. with assistance.
  • Walking or standing 80%, sitting 20%.

Server/Wait Staff

Reports to:                 Manager, Dining Services

Status:                        Non-Exempt (Hourly) 

Summary of Job Responsibilities:

Provide exceptional service to FHV residents and their guests in the Main Dining Room, Hunt Room and Café by maintaining professional, courteous and accurate service with a smile.  Set up of station prior to shift, maintain orders in station and clean up and proper re-setting at the end of shift.  Ensure proper billing and completion of all opening and closing duties.  

Essential Functions & Responsibilities:

  • Be ready for scheduled shift, well groomed, in full uniform (black dress pants and proper work shoes) including name tag and tie.
  • Complete assigned daily side work prior to beginning floor work/completion of shift.  Check table assignments and ensure they are properly set.
  • Greet guests at tables in a professional and hospitable manner, assure they have menus and obtain order while being attentive to resident/guest needs at all times.
  • Enter all orders, including drink orders into computer system, obtain and serve.
  • Ensure that checks are properly prepared and processed according to FHV Dining Policy.
  • Assure food is served efficiently and accurately. Periodically check if additional service is needed.
  • Assure checks are properly signed by resident, closed out to the correct resident apartment number and at end of shift review with Host for accuracy.
  • Wash hands immediately after visiting the restroom.
  • Wipe down check books, salt and pepper shakers, sugar caddies and chairs.
  • Regular punctual attendance, use time clock properly and take required breaks as instructed by policies and procedures.
  • Adhere to all safety policies including reporting all incidents immediately to your immediate supervisor, Food & Beverage Director or Human Resources.
  • Attend quarterly department staff meetings.
  • Complete assigned cleaning duties to keep dining room always in top shape.
  • Assist with other tasks such as cleaning the dining area and preparing silverware and condiments.
  • Channel all complaints to immediate supervisor immediately.
  • Push linen cart to Dining Room and unload linen into cabinets.
  • Be prepared to work in other areas of department (Main Dining Room, Hunt Room, Café, Room Service, Private Dining Room and/or special function)
  • Maintain an excellent and professional working relationship with immediate supervisor, fellow employees, Managing Director, Management Team, residents, guests and vendors.

  • 6 months to 1 year of food service experience in a residential and/or hospitality industry/setting.  Passion for hospitality and service. 
  • Strong team player: must be able to partner with servers, peers, and management to ensure an amazing guest experience.  
  • Current CPR Certification preferred

Physical Requirements:

  • Prolonged periods of standing and walking.  
  • Ability to lift and carry trays up to 30 lbs. including multi-gallon bucket of ice, soiled linen bags and room service cart
